Practice the "thumbing" technique before you begin shiatsu massage. Extend the thumb, do not bend it at the first joint or use the muscles in the thumb to press. Give gentle pressure with the thumb by leaning with body weight into the area being worked on. Hold the ball of the thumb flat. Stabilize the thumb by extending the fingers and resting them next to the thumb.

Locate the area on top of the foot between the fourth and fifth toe. Start at the web between the two toes and, using the thumb technique, gently apply pressure for 6-7 seconds. Slowly release move half an inch higher and repeat. Repeat moving half an inch higher until you can no longer get your thumb between the toe bones.

Locate the center of the top of the big toe. Using the same press and release method in Step 1, start at the top of the toe and continue down until you reach the web between the big toe and second toe.

Continue the press and release technique up the inside of the ankle.

Keep thumbing next to the shin bone up to the knee and stop.

Repeat the steps for the other foot and lower leg.
